Use the right form
The website uses Revenue NSW's June 2026 OPT 011 form, with its notes and contact page. Check that it is suitable for your contract and recipient. This is a subcontractor statement about workers compensation, payroll tax and remuneration, not a general-purpose statutory declaration.
Read the complete form and notes (PDF) or check the official Revenue NSW source.
Print and sign
The official form instructs users to print and sign. Choose Print and sign by hand to download a completed but unsigned form. Check it, print it, then sign and date it before giving it to your principal contractor. An unsigned download is not a signed statement.
On-screen signing
Only use on-screen signing after checking it is suitable and accepted for your circumstances. The website uses a drawn signature or an uploaded signature image you select; it does not provide a verified digital certificate, independent identity check or a server-held signature audit trail. A drawn image alone is not a guarantee of legal validity. Obtain appropriate advice if uncertain.
Confirm the facts each time
The signer must have authority to sign and be able to make the declaration truthfully. Review your workers selection, dates, insurance, wages, payroll tax and any subcontractor statements for the period. Saved defaults are starting values, not fresh confirmations. False statements can carry legal consequences.
Certificates and copies
Provide a Certificate of Currency separately where the form requires it. Keep the signed PDF and supporting documents in your own records. The form notes describe retention obligations for the receiving principal contractor, currently up to seven years. Check the rules that apply to you.
We do not store or send completed PDFs. An optional uploaded signature is stored privately with your business until you remove it and save. Download before closing or refreshing, and use your own secure delivery method.
